In order for us to prepare for an in-depth discussion about your child, a copy of the psycho-educational evaluation must be submitted in advance of the meeting.
Two teacher recommendations are required for Upper School Prep applicants. The forms should be downloaded, signed, and sent to your child's current English and math teachers.
Download a Release of Records Form and provide a signed copy of the form to your child’s current school. This form gives the school permission to send a copy of your child’s report cards from the last two years to Jemicy.
